To begin the application process, please provide the necessary information in the blanks provided.  After you have completed the form, click the submit button at the bottom of this page.  The information will be sent to our office.  You can either use a credit card or debit card to enroll immediately (Click here),  or you can remit your downpayment and all other forms to the address provided on the Enrollment Page. In making this application:

 

1.         As the parent or guardian of the above student, I understand the compulsory school laws of the state in

 which I reside and I accept full responsibility for my child’s enrollment with Faith Christian Academy for

 the purpose of  homeschooling my child.

2.         Those students who do not submit any course work over a period of two calendar months are dropped

 from the homeschool program and given a grade of W (Withdrawn) for each uncompleted course.

3.         If I decide to pay my bill with monthly payments, the payments are due by the tenth of the month.  A

$25.00 late charge will be added to my account after the tenth of the month.  If I miss two consecutive

 payments, my child’s lessons will be terminated and a grade of W (Withdrawn) will be given for each

 course.

4.         The school reserves the right to require the withdrawal of any student whose academic performance

            indicates an inability or unwillingness to meet the requirements of the school.

 

5.         You have read and agreed to the refund policy as stated below:

Refunds:
Up to the first seven calendar days following the official notification into our program, any credit balance remaining after the first monthly payment and any other fees such as book or postage fees are deducted from the credit balance will be returned.

Up to the first fourteen calendar days following official notification of acceptance into our program, fifty percent of credit balance will be returned.

Zero Refund: On the fifteenth calendar day following official notification of acceptance into our program there are no refunds, no matter what the reason or the number of lessons that you have received.

 

 

Processing of your application will begin within 24 hours of its receipt. You will be notified by e-mail that registration process is underway.   Your official notification of acceptance will be your email.
